Summary Care Records

Summary care records are a system allowing Doctors in hospitals to access limited information about, if they are involved in your care, such as:

  • Medication you are on
  • any allergies you may have
  • medications you have a reaction to

This information would be accessed only with your permission, unless you were say unconscious or otherwise unable to communicate.

If for some reason you do NOT wish to be included in this system (but have not already opted out) you should come into the practice and ask for an opt out form.

conversely there is also a system in place which will allow you to opt into allowing the people caring for you to have more information. This may be important if you have any special wishes for you care.
